SSHA General Petition Form

This petition is intended for students requesting course substitutions, concurrent enrollment, exceptions to School policies, or other academic requests not covered by another SSHA form on this page. After selecting Type of Request, review the required information and documentation, then submit this form with all requested materials.

You must submit a completed Undergraduate to take a Graduate Level Course form as in order for you request to be considered complete.

Your statement of support must speak to (1) why do you want to take the graduate level course, (2) how do you plan to be succesful in the graduate level course, and (3) how do you plan to balance all courses you are enrolled in.

Note: the form must be signed by the instructor of record of the graduate level course.

Use this form if you are a UC Merced student requesting to take a course at another institution during a Fall or Spring term while also enrolled at UC Merced.  

Before submitting this form, please be aware of the following information: 

  • You must be enrolled in the course at the other institution before submitting this request 
  • You must submit an unofficial transcript from the institution where you are enrolled 
  • Note: To request credit for coursework completed at another institution in a previous term (i.e. you have already received a final grade), select the Retroactive Concurrent Enrollment option of this form instead 
  • You must have completed at least one term at UC Merced and 12 UC Merced units

Statement Requirements

  • What institution you are enrolling at and what course(s) are you enrolling in 
  • What requirements you are hoping these courses satisfy at UC Merced 
  • Why you are enrolled at another institution while enrolled at UC Merced 
  • Why you are unable to complete the course(s) at UC Merced but are able to complete at another institution

Supporting Documentation

  • Required: Unofficial transcript or screenshot showing enrollment in the course 
    • Screenshot must include your name, name of college, course title, and term of enrollment

Use this form if you are a UC Merced student who has completed a course at another institution during the Fall or Spring term while also enrolled at UC Merced, and you are requesting to receive UC Merced credit.  

Before submitting this form, please be aware of the following information: 

  • You must have already completed the course at another institution and have a final grade 
  • You must submit unofficial transcript(s) from the institution you completed the course at

Statement Requirements 

Please explain the following:  

  • What course you completed and what institution you completed the course at 
  • Why you completed the course outside of UC Merced 
  • Why you did not submit this petition prior to enrollment at the other institution

    • In the case that an unofficial transcript cannot be obtained, a screenshot must include your name, name of college, course title, term of enrollment, and final grade

Use this form if you are requesting to enroll in more than 20 units in a regular Fall or Spring Term or more than 16 units in a summer term. 

Before submitting this form, please be aware of the following information: 

  • If you are applying to enroll in excess units (up to 20 in a Fall or Spring term or up to 16 in a Summer term) and meet the minimum 3.0 cumulative GPA or a 3.3 GPA in their last completed term (not including Summer), submit your request with the SSHA Petition for Excess Units form.

  • Why you need to enroll in more than 20 units in a regular Fall or Spring term or more than 16 in a Summer term

Use this form if you are a UC Merced student currently enrolled in a course at Merced College or are planning to enroll in a course at Merced College in the upcoming Fall/Spring term. 

Before submitting this form, please be aware of the following information: 

  • Complete the MC2UCM Cross Campus Enrollment Program Form 
    • Complete Home Campus Information 
    • Complete Section A 
    • Complete Section C (instructor signature is not required for processing at UC Merced) 
      • Must list “Reason” for taking course 
  • Note: To request credit for coursework completed at Merced College in a previous term (i.e. you have already received a final grade), select the Retroactive Concurrent Enrollment option of this form instead 
  • Note: You must submit an unofficial transcript showing proof of enrollment in the course at Merced College if the instruction has already started 
  • Incomplete information or documentation may delay the review or require resubmission of the petition 

NOTE: If approved, your form will be processed by both SSHA Academic Advising and UC Merced Office of the Registrar. Once both are complete, you will need to take a copy of your unofficial UCM transcript and the signed MC2UCM form to the Merced College Registrar for final processing.

Statement Requirements 
Please include the following:  

  • The course you intend to take at Merced College 
  • Why you were unable to complete the course at UC Merced 
  • How you plan to be successful in all coursework while enrolled at UC Merced and Merced College. 

Supporting Documentation

  • Required: Completed MC2UCM Cross Campus Enrollment Program Form
  • Only required if instruction has started: Unofficial transcript or screenshot showing enrollment in the course at Merced College 
    • Screenshot must include your name, name of college, course title, and term of enrollment 

Use this form if you are requesting to be exempt from one of your declared major or minor requirements. 

Before submitting this form, please be aware of the following information: 

  • This form is not for course exemptions for General Education requirements. If you are seeking an exemption from a GE requirement, review the policies and procedures for the GE Student Request Form.

Statement Requirements 
Please explain the following:  

  • Which requirement you would like to be exempt from 
  • Why you have been unable to fulfill the requirement before now 
  • Why you are unable to complete the requirement 

 

Supporting Documentation

Recommended, but not required: any documentation you feel supports your request to be exempted from the course requirement  

Use this form if you are requesting to complete coursework at another institution while on an approved Educational Leave (ELP) from UC Merced.  

 

Before submitting this form, please be aware of the following information: 

  • Per policy, students are not able to complete coursework elsewhere while on leave. This form is requesting an exception to that policy through the Dean.  
  • It is recommended that you confirm with your academic advisor that the coursework you intend to take is applicable to UC Merced requirements.

  • Why you are unable to enroll at UC Merced  
  • Why you are unable to complete courses at UC Merced but are able to complete courses at another institution 
  • Where you are enrolling 
  • List all course(s) you are enrolling in and what requirement(s) you are hoping to satisfy at UC Merced with those course(s)
